PHENIX Frequent Buyer Program Rules and Regulations:

1. You must register to be eligible. Please contact our Customer Service Department at
   800-767-0665 for your registration package or use our online form. Upon registration you will receive your first bonus stamp, redemption forms, and startup kit.
2. After registration, stamps will be issued for each consumable order over $200 excluding shipping and tax. For all orders falling into this category, you will receive one stamp for each $100 spent.
3. To obtain your Phenix stamps on eligible orders, you must mention your booklet number when ordering, or place your booklet number as a no charge line item on your purchase order; see the example below. For Web Site Orders be sure to include your Frequent Buyer booklet number in the Comments field at check out.
4. Stamps will be issued for consumable sales only. Specially quoted prices, equipment, and instrumentation sales are not eligible.
5. Frequent Buyer Stamps are shipped with the order with which they were placed.
6. Program is based on current catalog pricing or published contract pricing at your institution. Specially quoted prices do not qualify. The stamps cannot be combined with free items or promotional gifts advertised by PHENIX.
7. Redemption of stamps is subject to verification. Redeem your stamps by returning the stamps with a redemption form.
8. Bonus items will be shipped within 30 days of receipt of redemption request.
9. If requested item is not available, PHENIX reserves the right to offer an item of equal or greater value. PHENIX will make an effort to contact you before making changes to your order.
10. This program is void where prohibited by law or organization's rules. It is a customer's responsibility to verify that participation in this program is in compliance with Customer's Organizational Policies.
11. Frequent Buyer stamps have no cash value and are not exchangeable for cash.
